Intermediate care, reablement and hospital discharge

A key element of P&EI is a whole systems approach to supporting people to live independently and the prevention of premature admission to residential and nursing homes and acute hospital care and facilitating early discharge from acute settings, through good jointly planned and managed intermediate care services, crisis response, reablement and good discharge planning.

This supports both best practice for service users and efficiency for the whole system.
