Practice Based Commissioning Pilot Schemes

Two Carers Practice Based Commissioning (PbC) Pilot Schemes are underway in the region. PbC consortia were asked to submit proposals on how they would enhance services within primary care settings to secure cost effective improved outcomes for patients with long term conditions through support for carers. This recognises the pivotal role that primary care clinicians have in identifying and supporting carers, both in terms of their own health and in helping patients to maintain their support network in the community.
